Mara L.’s path to revision after a change of trade.

Mara returned to AREOLA to revise a steel system that had performed well in every respect except the one that had come to matter most in her work.

“It held everything I asked it to hold, right up until the day I changed what I was asking.”

Mara came to the Boston center for a revision, not a first replacement. Her original steel system had been placed nine years earlier at another institution and had performed, by her account, without a single complaint. What changed was her work: a move from light-gauge ductwork into structural fabrication, and with it a move from sheet screws to self-tapping fasteners half again as heavy.

Her specialist treated the complaint as a specification problem rather than a clinical one. The existing plates were a grade appropriate to what she had been carrying and were simply not rated for what she was carrying now. A materials review established that a revision to a higher-grade magnetic interface was reasonable, and that the tissue would tolerate it.

Mara is careful to say that nobody rushed her toward the revision. “They were very clear that I could keep what I had and just put the heavy stuff in a pouch like everybody else,” she says. “I appreciated that they said it. I also knew I was not going to do it.”

She completed her first Annual Plate Check in the spring. The documented plan notes the fastener range the system was specified against, which she says she has already referred to twice when discussing tooling with a supplier.
