Requesting your record

Patients may request a copy of their AREOLA record, including the documented plate plan and design history, through Medical Records Intake at 334-209-7090 or records@areolainstitute.org. A written authorization is required, and requests are confirmed before anything is released. Email is not a secure channel: please do not send records, health information or other personal details by email.

Sending records to AREOLA

Outside records may be sent to Medical Records Intake in advance of a consultation. Sending them early is helpful: a coordinator can confirm what arrived, identify what is still missing and make sure the specialist has reviewed the file before you are in the room.

Release to another clinician

Records are released to an outside clinician only with your authorization, and you may limit what is included. AREOLA does not release records to employers, insurers or family members without a specific authorization or a legal requirement to do so.
