What the card records
The card carries your registered system, material, attachment type, the center that holds your record and the institutional contact line. It is deliberately brief: it exists so that another clinician can identify the system quickly, not to summarize your care.
When it is useful
Imaging departments, security screening, unfamiliar clinicians and urgent care visits are the situations patients most often describe. The same information is available in MyAREOLA, but a physical card does not depend on a charged phone or a signal.
Replacing a card
Patient Services reissues cards at no charge; a replacement is usually mailed within a week. Details also change — a revision, a new interface, a transfer between centers — and reissuing the card is a routine part of updating your record.
