Patients who arrive with a marked-up guide tend to have more focused consultations. Coordinators can send a guide ahead of a scheduled appointment, and nursing will review any section of one with you during a visit.

What forms do not do
Completed forms are brought to an appointment or sent through Medical Records Intake using the instructions supplied by the receiving center. This page is informational and is not a record-transfer channel.
Alternative formats
Materials can be provided in large print or another format on request through National Patient Services, and interpretation is arranged at no cost to the patient.
