Completed forms are brought to an appointment or sent through Medical Records Intake using the instructions supplied by the receiving center. This page is informational and is not a record-transfer channel.
Materials can be provided in large print or another format on request through National Patient Services, and interpretation is arranged at no cost to the patient.
