Measures that follow the journey

The report focuses on 30-day follow-up, understandable answers to care questions, complete plate-plan documentation and participation in recommended Annual Plate Checks.

2025 network results

Reported results include 97.4% 30-day follow-up, 96.1% of surveyed patients reporting understandable answers, 99.2% complete plate-plan documentation and 84.7% Annual Plate Check participation among eligible patients.

Measures are reviewed across the network and by center. Variation prompts clinical, operational or communication review and is not treated as a stand-alone conclusion about care.

Patient experience

Survey, navigator and advisory-council feedback identify themes in preparation, shared decisions, travel, recovery communication and continuity.
